TELEHEALTH CONSENT FORM

Last Updated: March 1, 2026 Telehealth refers to the delivery of healthcare services using communication technologies (such as the internet or mobile devices) to assist with diagnosis, consultation, treatment, education, care coordination, and patient self-management.

By acknowledging my consent below, I confirm that I understand and agree to the following:

• I understand that Meds1 provides Telehealth consultations through video calls, phone calls, and asynchronous communication, and that my healthcare provider will not be physically present with me during these sessions.
• I recognize that Telehealth services may involve certain risks, including but not limited to technical disruptions, delays, or unauthorized access. I understand that either I or my provider may end the session if the connection quality is inadequate.
• I understand that I may choose to receive care through an in-person visit instead of Telehealth, and that I am voluntarily opting to participate in a Telehealth consultation with a Meds1 provider.
• I acknowledge that a consultation with a licensed clinician is necessary, and I understand that my request for treatment may be declined based on their professional judgment.
• To help protect the privacy of my health information, I agree to attend my Telehealth session from a private setting, and I understand that my provider will also be in a secure and private environment.
• In case of an emergency, I understand that my Telehealth provider may advise me to seek immediate in-person care, such as visiting the nearest emergency room.
• By proceeding, you consent to a telehealth consultation with an independent licensed clinician. A professional medical fee of $20 is included in the amount paid.

By acknowledging below, I confirm:

• that I have read this document or had it explained to me;
• that I understand the potential risks and benefits of participating in a Telehealth visit;
• that I have had the opportunity to ask questions and that all my questions have been answered satisfactorily.

CONSENT TO TEXT OR EMAIL USAGE FOR APPOINTMENT REMINDERS AND OTHER HEALTHCARE REMINDERS

By signing below, I agree to receive text messages at my phone number and/or emails for appointment reminders, general health notifications, and related information. I understand that this consent will remain in effect for future communications unless I revoke or modify it in writing. I also acknowledge that these communication methods may not be fully secure for sharing sensitive or personal information.
